- All rates are 2026 CMS national average Medicare non-facility reimbursement from the Final Physician Fee Schedule, effective January 1, 2026.
- Actual reimbursement varies by geographic location (GPCI), individual payer contracts, and Medicare Administrative Contractor (MAC) policies. Commercial and Medicaid payers set their own rates independently.
- CPT 99445 and 99454 are mutually exclusive per patient per month. Bill 99445 for 2–15 days of data; 99454 for 16–30 days.
- CPT 99470 and 99457 are mutually exclusive per patient per month. 99470 cannot be billed concurrently with 99457.
- CPT 99458 may be billed a maximum of two times per patient per month (once for 20–39 min total; once for 40+ min total).
- CPT 99453 is a one-time code per patient at device setup/enrollment. It is not recurring.
- RPM requires: a qualifying chronic condition, documented patient consent, an FDA-cleared monitoring device, and Medicare Part B eligibility or a participating commercial payer contract.
- ACO and Health Plan projections represent estimated program-wide RPM reimbursement and may involve direct contracts, shared savings, or delegated care models that differ from traditional fee-for-service billing.
